Form improvements - used once then deactivated/deleted

When a form is created via import, a custom form must be created that is not a template as this form will only be used once as the content is different on all versions.

The solution will be to use a standard empty form template where we change the name, content, questions, etc. based on data located in excel sheets provided by Nitton93.
